During your initial visit, you will have to complete the Patient Registration Form as well the Medical History Form.  You can simply click on the link to print out the form.

You may review our HIPAA Notice of Privacy Practices or our HIPAA Notice for the Release of Identifying Health Information Policy as well.  You can print it out and bring in a signed copy of both forms or you can simply sign it at our office during your initial visit.

Finally, it is imperative you review our Financial Policy.
